I need a webpage that has 6 input fields. Website visitors input personal information into 6 fields. When the visitor clicks a button ("E-mail me my form"), a PDF is generated with the information from those 6 fields and sent to the e-mail address given. The information from the 6 input fields is also saved in a database.
